I am no longer running the full test service and you can now order most of your Lab Tests here. However, if you can't find a test you need, just send me this form and I am happy to either source it for you or order/authorise it for you myself. I've added a tiny admin charge to cover my time to source, authorise, have a quick look at the results and email them to you with a brief idea of what they mean and what to prioritise.

Paying for tests: most of the time, the lab will send you an invoice or payment link once I've authorised it, or I will send you a payment link if not.
